Twelve one to one sessions, each built on the last, working through communication, behavior, relationships, or leadership depending on what brought you here. It is long enough for a pattern to be identified, altered, tested, and reinforced, and short enough to have a visible end.

The Five Focus Areas That Shape the Program

Every engagement moves through the same five focus areas, though the time spent on each varies. Self-awareness comes first, because nothing changes while you are still arguing with the description of the problem, and this stage establishes what you do, when, and under what conditions. Accountability follows, which is the point where you separate what belongs to the environment from what belongs to you, without either absorbing everything or dismissing all of it. 

Skill development is the practical middle: specific techniques for the specific situations you face, not general principles. Practical application takes those skills into your actual workplace between sessions, which is where most of the real work happens. Reflection closes the loop, reviewing what shifted, what resisted, and what still needs attention. The sequence matters. Physicians who skip ahead to technique before doing the self-awareness work tend to apply the technique correctly and get no result, because the underlying pattern is still running.

What Actually Happens in a Single Session

A session runs as a focused conversation, and it begins with what has happened since the last one. You describe a real exchange, decision, or conflict from your week, in enough detail that the specifics are available rather than a summary of them. From there the work depends on the material. Sometimes the session unpacks what occurred and why it went the way it did. Sometimes you rehearse an upcoming conversation aloud, several times, adjusting the wording until it sounds like you rather than like a script. 

Sometimes the DiSC profile gets pulled back out because the same behavioral collision has appeared again in a different setting. Each session usually ends with one thing to try before the next, deliberately small enough to be realistic on a working week. There is no reading, no written homework, and no exercises to complete. Your clinical life supplies all the material the program needs, which is why physicians can manage this alongside a full schedule.

How Twelve Sessions Are Spaced Around Clinical Work

Cadence is agreed rather than imposed, and it is built around your rotation rather than a fixed weekly slot. Sessions commonly sit two to three weeks apart, which leaves enough time for a new approach to meet a real situation and produce something worth discussing, without so much distance that continuity is lost. A twelve-session program therefore typically runs across several months rather than several weeks. Gaps are expected. 

A heavy stretch on service, a night float block, or annual leave will interrupt the rhythm, and that is treated as normal rather than as a failure of commitment. Sessions are held virtually, which is what makes this workable: you can meet from a home office, a call room, or a clinic between blocks with no travel attached. Physicians in Seattle, Portland, and Los Angeles are all in the same time zone, which keeps scheduling straightforward, and arrangements further afield are perfectly manageable.

When the Program Extends to 24 Sessions

Twelve sessions suit most situations, and many physicians finish there with what they came for. The twenty-four-session extension exists for engagements where the material is genuinely larger. That includes long-standing patterns formed over decades of practice, situations where a physician is rebuilding a working relationship with an entire unit, leadership transitions that unfold over a year, and cases where an institution has asked for a longer period of demonstrated change. 

The extension is a decision made during the program rather than at the start, usually somewhere around the eighth or ninth session, when it becomes clear whether the work will be complete. Nobody is signed up for twenty-four sessions on day one, and the extension is not treated as a default. Where twelve is enough, twelve is what you do.

Fees depend on the length of the engagement and whether a physician or an organization is arranging it, so they are discussed directly in the introductory conversation rather than published as a single figure.
