Why Do Strong Clinicians End Up With Communication Problems?

Medical training rewards speed, certainty, and independent judgment. Those same qualities can read as impatience or dismissiveness at the bedside and in the workroom, and almost nobody gets corrected until something formal happens. The data show the pattern. Vanderbilt researchers tracking unsolicited patient complaints found that fewer than 10 percent of physicians accounted for more than half of all complaints, while 37 percent had none recorded against them at all. Complaints cluster, and the physicians in that cluster are usually the last to know.

The 12-Session Physician

This is the format all the coaching runs through. Twelve sessions allow the sequence of self-awareness, accountability, skill development, practical application, and reflection to unfold at a workable pace. An extension to twenty-four sessions is available where more time is warranted. Session timing is arranged around call and clinic rather than fixed in advance.

Few conversations expose communication habits faster than sitting with a family to discuss a poor prognosis. Time pressure pushes physicians toward efficiency, and efficiency in that room reads as coldness. Candello data show that claims involving provider and patient communication failures most often arise in ambulatory settings, at 60 percent, which places ordinary conversations rather than emergencies at the center of the risk. 

In coaching I help you structure these meetings deliberately: how to open without a preamble that raises false hope, how to deliver the central fact in plain language and then stop talking, and how to manage a room where people are reacting at different speeds. Communication Coaching for Teaching, Supervision, and Resident Feedback

Physicians in academic settings communicate under observation constantly, and the same remark that a peer would shrug off can land very differently on a second-year resident. Trainees are also more likely to report what they experience, which means teaching interactions are frequently the source of formal concerns. I work with attending physicians and program faculty on giving correction that is specific and usable, on separating the clinical error from the person who made it, and on recognizing when authority is doing the talking instead of the teaching. 

Vanderbilt research has found that residents rated lower for professionalism and interpersonal communication went on to attract more unsolicited patient complaints in their first year of independent practice, so what gets modeled in supervision has a long tail. A growing share of physician communication never happens out loud. Being shown your complaint data or your experience scores is a particular kind of unpleasant, especially when the comments feel unfair or describe a day you barely remember. The evidence suggests it is worth engaging with anyway. Vanderbilt reports that across its peer intervention program, 80 percent of more than 1,600 physicians who received a peer-delivered conversation about their complaint pattern responded favorably, with fewer subsequent complaints and lower malpractice claim experience. 

My part is to help you read that data without either dismissing it or taking it as a verdict on your worth as a physician. Sessions work through what the comments have in common, which of those threads you can influence, and what a realistic change looks like for someone carrying your patient volume.

Frequently Asked Questions About Physician Communication Coaching

Does my department or chair need to know I am doing this?
Not if you are arranging the coaching yourself. A self-referred engagement is a private professional decision and nothing is reported to your institution. If your organization is arranging and funding the coaching, they will know it is taking place, though the content of sessions still stays private. The difference is simply who initiated it.
